Risk Management is responsible for administering the City of Sarasota’s insurance programs. Areas of our self-insurance programs include General Liability, Fleet Liability, and Workers’ Compensation. Additionally, Risk Management administers Property Insurance, Professional Liability, Safety, and Loss Prevention.

Risk Management services are dedicated to maintaining a safe, clean and healthy environment for our citizens, visitors and employees.

File a Claim for Property Damage or Injury

To file a claim for Property Damage or Injury complete and return this form: Incident Questionnaire. Please be aware that any person who knowingly and with intent to injure, defraud, or deceive any insurer, files a statement or claim or an application containing any false, incomplete, or misleading information is guilty of a felony as established under Florida Statute 817.234.

You may submit your completed form along with any supporting documents and photographs by email to Risk@sarasotafl.gov, by fax to 941-951-3635 or you may send by mail to:
